Is Southwest Dental Plan dental
insurance?
Southwest Dental Plan is not
insurance. It is a dental plan in which participating
dental providers have agreed to provide Plan members with dental
services at reduced fees. |

Who is eligible to enroll in
Southwest Dental Plan?
Any individual and spouse are
eligible, along with dependent children to age 19, or age 23, if
full time students. Membership may be continued for a
child over age 19 who is incapable of self-sustaining employment
by reason of developmental or physical handicap.
Membership is available to all individuals without regard to
age, income or employment status.
How do I join Southwest Dental
Plan?
Just fill out our subscription
form and mail it in, or sign up online. Get more

When does the plan go into
effect?
Membership in Southwest Dental
Plan can become effective the same day the enrollment
application and membership fee are received, although it
normally takes 5 to 7 working days to process your application.
You will receive your membership information package and card
shortly after that. If you have an emergency before
receiving your membership package, simply call (480) 768-3956
and we will confirm eligibility.
What if I have another dental
plan?
Southwest Dental Plan may be used
after you have exhausted all your benefits from your PPO or
indemnity plan.
Is specialty care included?
Yes. Southwest Dental Plan
covers specialty care (endodontics, periodontics, oral surgery,
TMJ, and orthodontia). Fees are listed in the Specialty
Care Section of the
Fee Schedule (167kb PDF).
Summary of Benefits
All dental
treatments are covered at the fees listed in the
Fee Schedule (167kb PDF). Any treatment not listed will
be performed at a 20% discount to the provider's fees.
Only procedures performed at contracted offices are covered
under this plan.
Limitations and Exclusions

1. |
Refunds on
purchases of the Southwest Dental Plan will not be made after 45
days of the date of purchase of the Southwest Dental Plan.
If the plan is cancelled within the first 45 days, a $10 charge
will be deducted from the refund and the patient will be billed
the usual and customary charges for all procedures performed
prior to the cancellation. |

Prescriptions
and over-the-counter medications are not covered. |

3. |
Services
provided by non-participating dental providers are not a covered
benefit. |

4. |
Dental care
payments covered by any other plan or agency can not be
duplicated or combined. |

5. |
Loss or theft of
any dental appliance is not covered. |
|
6. |
Failure to
follow prescribed dental procedures may incur additional fees. |
|
7. |
Out-of-area
emergencies are not a covered benefit. |
|
8. |
Only procedures
performed at contracted offices are covered under this plan. |
|
9. |
If a member
becomes ineligible during the course of treatment, benefits
under this plan cease and the member becomes fully responsible
for continuing treatment. |
